If you read the story of my patient Korhan in Izmit, you will remember. With the treatment we gave after six times micro TESE, pregnancy occurred naturally even though sperm was not in the semen.
In fact, I was with them on their birthday a while ago and we shared photos on my instagram account.
Another case is our patient Adnan S. from Bilecik.
According to the reports he had before Mr Adnan came to us, if he was correct, he was a case in which sperm could not be found when he was searched once with an operation, although he showed immotile sperm output several times.
In our analysis system, we could not see sperm and the system was very backward. The table was as far back as the Sertoli only cell syndrome. In fact, he did not take the treatment as we wanted. Since Adnan Bey, who was supposed to come five times in a year, came to the controls less often, I warned him in a friendly language, “This will not go like this, it is not right” and emphasised the issue of smoking and sports.
This time the advice must have worked because we were very happy when we received the news of spontaneous pregnancy, albeit delayed. Although I invited him to the azoospermia awareness meeting in March 2017 together with Korhan, Korhan came but he could not come due to his busy schedule. If I look at the progress of the semen in the last analysis, it was still behind in terms of sperm formation stage. However, this miraculous result, which developed after the last four months of hard work, raised our hopes.
I think that during the treatment of azoospermia, it seems that we will leave our classical logic of searching for sperm cells. These developments seem to show us that we are moving towards a changing sperm structure instead of sperms like before. For this reason, since not everyone can be as lucky as Korhan and Adnan, I believe that we are approaching good results by careful cell selection, searching and using the most genetically ready or in other words the most mature spermatid, performing micro ICSI with that spermatid, providing genetic control in the embryo that is resistant to the fifth day (currently PGD NGS) and transferring it at the appropriate time and ground.
I also know that an azoospermic patient had a miscarriage a while after seeing her natural pregnancy, gestational sac, increase rates in pregnancy tests, etc. with all the details. He even emphasised that such a thing developed after reflexology and cupping. His father named this friend Dursun. I think he’s in dursun mode for now. Although he saw all these developments, he somehow broke away from the treatment. I guess he will fly back again. I think my invitation message went to him.
Come in, sir, our door is open…
